2011/2012 Consumer Service Questionnaire Rules & Regulations

Prize: One of 3 Sunquest $1,000 Future Travel Vouchers

In appreciation for completing the Consumer Service Questionnaire, all participants will be entered in a draw for a chance to win one of three $1,000 Future Travel Vouchers from Sunquest.

To be eligible for this Contest, an individual must:

• be a legal resident of Canada ;
• be of the age of majority or older in his/her province or territory of residence at the time of entry
• submit the Sunquest Consumer Service Questionnaire by May 31, 2012.
  

This Contest is void where prohibited and thus not open to residents of Quebec.

On July 31, 2012 three winners will be determined through a random drawing from all completed questionnaires. The odds of winning are solely dependent on the total number of completed questionnaires. Winners will be notified by August 10, 2012.

The prize must be accepted as awarded. Subject to the right of Sunquest, in its sole discretion, to substitute a prize or a component of a prize of equivalent value without prior notification, no cash or other prize substitution is permitted.

The selected potential winner will be contacted by e-mail or telephone. Before being declared a winner, the selected winner must first correctly answer, unaided, a time-limited mathematical skill-testing question posed by e-mail or telephone at a mutually convenient time. If the selected entrant cannot be contacted by mail or telephone within 10 business days of the draw date, said selected entrant will be disqualified and another entrant will be randomly selected until such time as contact is made with a selected entrant or there are no more eligible entries, whichever comes first. If the selected entrant cannot use or chooses not to use the prize, that prize will be deemed to be forfeited and no substitution or other compensation will be awarded.

Sunquest Future Travel Voucher Terms and Conditions:

• The voucher entitles the winner to a total of $1,000 CDN in travel toward a Sunquest all-inclusive holiday package. Not applicable on Sunquest cruise bookings.
• Any expenditure exceeding $1,000 CDN is payable by the prize winner at time of booking.
• The voucher may be applied to one vacation departure date only, booked in one room .
• Prize must be accepted as awarded. The value of the voucher cannot be exchanged for cash, is non-transferable, non-refundable, and cannot be substituted or used to pay taxes.
• No refunds or credits for future travel will be issued for any unused amount. 
• The travel voucher is valid until December 15, 2013.
• Travel is subject to space availability. Black out periods and holiday restrictions apply: Christmas, New Year¿s (December 21 - January 2 departures), February 10-27 departures, March/Spring Break, Easter Week, Construction Week (Quebec) and others may apply.
• Once the holiday package arrangements have been confirmed no changes will be permitted.
• All bookings are subject to 100% cancellation penalties.
• Flight times, flight itineraries, carriers and aircraft types are subject to change with or without notice.
• Documentation(s) is the sole responsibility of the passenger(s). The passenger(s) at his/her expense must obtain all documentation(s) required by all relevant government authorities (this will vary depending on Citizenship status).
• Travel insurance including medical insurance is not included and is the sole responsibility of the passenger(s).
• Expenses incurred for items of personal nature, local excursions and any gratuities are the responsibility of the passenger(s).
• Sunquest shall not assume responsibility for any refunds, losses, damage, costs or expenses arising out of the following: mechanical breakdown, government action, weather, strike or any other action beyond Sunquest¿s control; denial of access to the carrier due to contravention of the conditions of carriage or improper travel documentation; failure to clear customs or missed flights or connections.

Personal Information:

All personal and other information requested by and supplied to Sunquest for the purpose of the survey must be truthful, complete, accurate and in no way misleading. Sunquest reserves the right in its sole discretion to disqualify any entrant that they believe to be abusing the rules. Such abuse includes entering false information and entering more than once.

Employees of Sunquest, its affiliates, advertising and promotional agencies, their respective affiliates, subsidiaries, related companies, as well as members of immediate family (defined as spouse, children, siblings and parents, regardless of where they reside) and household (whether related or not) of such employees are not eligible to win.

The entrant, by entering the Contest and voluntarily providing his or her personal information, agrees to the Sponsor’s collection and use of the Entrant Information, to administer the Contest. No other use or disclosure will be made of Entrant Information: Information may be used and disclosed by the Sponsor, its affiliates and their respective designees as described below. All entries become the property of Sunquest upon receipt and will not be returned or acknowledged. By submitting the entry form for the Contest, the entrant agrees as follows: (i) to abide by and be bound by the Rules, including the eligibility requirements, failing which, an entrant may be disqualified; (ii) to abide by and be bound by all decisions of Sunquest and judges, whose decisions shall be final and binding in all matters relating to the Contest; and (iii) by accepting a prize, to allow the use of the entrant's name, likeness, city and province/territory of residence, statements, voice and biographical data by Sunquest and its affiliates and their respective designees in perpetuity throughout the world in connection with the Contest in any media or format, whether now known or hereafter developed, including but not limited to the World Wide Web, at any time or times without further compensation, permission or notification, and to provide written certification to that effect.

Sunquest reserves the right to suspend, alter or cancel this promotion for any reason what so ever without prior notification.
